Eligibility
- 1st of the month following 30 days of employment
Eligible dependents (spouse and children)
- Children who were covered under this Contract as an Eligible Dependent on or after May 1, 2010, are allowed to continue coverage up to age 26, notwithstanding marriage, absence of student status, or financial dependency on the Subscriber. In addition, any Subscriber whose coverage is first effective June 1, 2010 or later may enroll his/her child who is under age 26 and who was not previously covered under this Contract, notwithstanding marriage, absence of student status, or financial dependency on the Subscriber.

Plan Option 102:
- DSI will cover 80% of employee premium up to $600
- DSI will cover 50% of employee & spouse, employee & children, or employee & family premium up to $850
or Plan Option 306:
- DSI will cover 80% of employee premium up to $350
- DSI will cover 50% of employee & spouse, employee & children, or employee & family premium up to $500

Eligibility
- You must have completed 6 months of service and be at least 21 years of age.
Enrollments
- Quarterly on January 1, April 1, July 1, and October 1 after eligibility requirements are met.
Employee Contributions
- Up to $18,000 (2015); with a catch-up limit of $6,000 (2015) for those over 50
- Pre-tax, after-tax, or both
Employer Contributions
- Dollar-for-dollar up to 3% of annual compensation; plus 50% on the next 2% of your compensation.
Vesting
- Safe Harbor contributions and earnings 100% immediate. Employer’s profit sharing contributions and earning will follow vesting schedule.
Loans
- Not permitted by law.
Distributions
- May only occur in the event of: retirement, death, disability, termination of employment, hardships.
